How to register
To register as a permanent resident please come in and make an appointment to see the doctor.
You will need to fill in a registration form - you can pick that up when you come or you can download one here and bring it with you. You should also bring a form of identification - acceptable forms of ID are listed on the registration form - and, most important if you have repeat prescriptions, a copy of your prescription re-order form. We will obtain your records from your previous UK GP.
To register as a temporary resident just fill in a form when you come to the surgery. We will send a record of your treatment to your own GP.
Access to Medical Records
If you wish to see your records, or you need a copy of a letter or results to take to outpatients, just ask.
If you would like a complete copy of your records please ask.
We regret that out current IT system does not support viewing your medical records over the internet.
